Also known as the Titan 2+2, this was a United Tech concept for a Titan with four boosters. I have a report on this concept available for sale here.

Was it actually supposed to launch on 2 of the 4 solids only, or is this an artist’s error?

It’s correct. The boosters would be fired sequentially *or* simultaneously, dependant upon payload, upper stages and trajectories.
