Taken from the same brief presentation as this twin-fuselage C-5, this is a notional concept for much the same thing but using 747 components. Clearly it was not designed to any great level of detail. It would have required a vast level of redesign of the 747 as compared to the twin-C-5. Moving wings from the bottom of a fuselage to the top ain’t exactly a trivial operation.

The 747 had a lot of input from Boeing’s losing design for the C-5 contract, which did have high-mounted wings – so I assume they polished off those plans for this aircraft.
I suspect the main landing gear would need a bit more work. If I was going to raise the wings, I’d probably but the main gear in sponsons using as muc as I could of the existing structure rather than redesign the whol gear installation. I still think a twin C-5 would make more sense.
